Prolieve® Thermodilatation
What Is Prolieve® Thermodilatation?
Prolieve® Thermodilatation is a minimally invasive medical procedure designed to alleviate symptoms associated with benign prostatic hyperplasia (BPH), commonly known as an enlarged prostate. The procedure uses both microwave energy and a dilating balloon to heat and widen the prostatic urethra. This dual-action approach helps to reduce prostate obstruction and ease urinary symptoms without the need for more invasive surgical techniques.
When Is Prolieve® Thermodilatation Needed?
Prolieve® Thermodilatation is primarily recommended for men experiencing moderate to severe symptoms of BPH who have not responded well to conventional medical therapies. Indications for considering this treatment include:
- Persistent Urinary Symptoms: Patients experiencing urinary frequency, urgency, weak stream, and difficulty starting urination despite using medication.
- Alternative to Invasive Surgery: This procedure is suitable for those who wish to avoid more invasive surgical procedures, such as transurethral resection of the prostate (TURP) or prostatectomy, which typically require longer recovery times and carry higher risks of complications.
- Risk of Urinary Retention: Prolieve® is also considered for patients at an increased risk of urinary retention, where the person is unable to empty their bladder fully, causing frequent discomfort and potential for kidney damage.
Benefits Of Prolieve® Thermodilatation
Prolieve® Thermodilatation offers several benefits for patients suffering from BPH, making it an attractive option for many. These benefits include:
-
Minimally Invasive Nature
Unlike traditional surgical options, Prolieve® Thermodilatation is minimally invasive. This approach significantly reduces the recovery time and allows most patients to return to their normal activities shortly after the procedure.
-
Symptom Relief
The treatment effectively alleviates symptoms of BPH, such as urinary frequency, urgency, weak stream, and the feeling of incomplete bladder emptying. Patients often experience significant improvement in their urinary flow and overall comfort.
-
Reduced Risk Of Complications
As a non-surgical treatment, Prolieve® Thermodilatation may carry a lower risk of complications compared to surgical interventions like TURP or open prostatectomy. Common surgical risks such as bleeding, infection, and issues related to anaesthesia are markedly reduced.
-
Outpatient Procedure
The procedure can be performed in an outpatient setting, meaning patients do not need to stay overnight. This aspect particularly appeals to those who prefer a treatment that minimally impacts their daily lives.
-
No General Anaesthesia Required
Prolieve® Thermodilatation is typically performed under local anaesthesia, which is safer than general anaesthesia, especially for elderly patients or those with significant medical comorbidities.
Preparing For Prolieve® Thermodilatation
Preparing for Prolieve® Thermodilatation involves several steps to ensure the procedure is safe and effective. Patients are typically advised to:
- Consultation and Evaluation: A thorough consultation and evaluation with a urologist is important before the procedure. This includes a detailed medical history and a physical examination to confirm that Prolieve® Thermodilatation is the most suitable treatment option based on the patient’s symptoms and health status.
- Medication Review: Patients should discuss their current medications with their doctor. Some medications, especially those that affect blood clotting, such as aspirin or anticoagulants, may need to be adjusted or temporarily discontinued before the procedure.
- Fasting Requirements: Depending on the specific protocols of the treating facility and the type of anaesthesia used, patients may be required to fast for several hours before the procedure.
Procedure Overview
Prolieve® Thermodilatation is an outpatient procedure typically taking 45 to 60 minutes to complete. The process involves several key steps:
Insertion of the Device
The Prolieve® device, which includes a catheter equipped with a microwave generator and a thermally controlled balloon dilatation balloon, is carefully inserted into the urethra. The device is positioned so that the balloon sits within the prostatic urethra, where the enlargement is located.
Activation of Microwave Energy
Once in place, the device emits controlled microwave energy to heat the prostate tissue. This energy helps reduce the size of the enlarged prostate by causing tissue shrinkage.
Balloon Dilatation
Simultaneously, the balloon dilates, gently stretching the urethra and further helping to relieve the obstruction caused by the enlarged prostate.
Monitoring
Throughout the procedure, the patient’s vital signs are monitored, and the temperature of the prostate is carefully controlled to ensure safety and effectiveness.
Completion
After the treatment is completed, the device is removed. The patient is observed for a short period to ensure there are no immediate complications before being allowed to go home.

Potential Risks And Side Effects
While Prolieve® Thermodilatation is generally considered safe and less invasive than traditional surgical treatments for BPH, it is not without potential risks and side effects. Patients considering this procedure should be aware of the following possible complications:
- Urinary Tract Infections (UTIs): Inserting any instrument into the urethra can introduce bacteria, potentially leading to infections. Symptoms of a UTI may include pain, burning during urination, and frequent and urgent needs to urinate.
- Mild Discomfort Or Pain: Some men may experience mild discomfort or pain during or after the procedure, particularly around the treated area. This discomfort is usually temporary and can often be managed with over-the-counter pain relievers.
- Urinary Retention: In some cases, swelling caused by the treatment or the initial irritation can lead to difficulty urinating or complete urinary retention. This condition may require temporary catheterisation.
- Hematuria (Blood in Urine): It is not uncommon for patients to notice a small amount of blood in their urine immediately following the procedure. This should resolve on its own within a few days.
- Transient Urinary Symptoms: Following the procedure, some patients may experience transient urinary symptoms such as urgency, frequency, or incontinence. These symptoms typically improve as the healing process progresses.
- Rare Complications: Although rare, more serious complications such as severe infection, significant bleeding, or damage to the urethra can occur. Immediate medical attention is required if any of these severe symptoms are observed.
Recovery And Post-Procedure Care
Recovery after Prolieve® Thermodilatation is generally quick, with most patients able to return to their normal activities within a few days. Appropriate post-procedure care is important to ensure a smooth recovery and to minimise the risk of complications. Here are the key aspects of post-procedure care:
-
Activity Restrictions
Patients are typically advised to avoid strenuous activities and heavy lifting for a few days post-procedure to prevent any undue stress on the treatment area.
-
Hydration
Increasing fluid intake is recommended to help flush out the urinary tract and aid in recovery, especially if minor bleeding occurs.
-
Pain Management
Over-the-counter pain relievers may be recommended to manage any discomfort following the procedure. If pain persists or worsens, it is important to consult a urologist.
-
Monitoring For Complications
Patients should monitor for signs of complications such as severe pain, fever, persistent or heavy bleeding, or difficulties in urinating. Immediate medical attention should be sought if any of these symptoms occur.
-
Follow-up Appointments
Follow-up visits with the urologist is important to monitor the healing process and evaluate the treatment's effectiveness. This may include urinary flow tests and discussions about any residual symptoms.
-
Catheter Care
Proper care and hygiene are vital to prevent infection if a catheter is temporarily placed due to swelling or difficulty urinating.
